The CSB just released a video on the August 23, 2010 Alabama ammonia release. While not perfect, it is a worthwhile video to watch as it might just make you question how you should deal with the type of power-failure issues that were a part of this release.
In conjunction with the PHA, a properly prepared / followed SOP could easily anticipate these events and mandate safe steps to ensure that these issues could be addressed safely.
I think the recommendations made by the CSB makes are too broad, but you can certainly use this video and the accompanying Safety Bulletin to start a conversation on how these issues can be safely addressed in your facility!
